I just emailed the address I gave you for requesting a waiver to get information and got a form email back.  That doesn’t mean they won’t answer, just that they are probably slammed at the moment.

It is the form with the required information.  I would fill this out carefully to fully support your request.  If you are already covered by the list of essential businesses, please don’t fill one of these out.
