Lending Library Surveys
Lending Library users are required to return a survey with each kit they borrow (this includes equipment and expendables). One survey must be filled out per group of students using the kit. Our funders require the data from these surveys in an annual report. Although we provide at least one hard copy of the survey with each kit, the surveys are accessible below in case anyone needs extras:
